Payment Policy
Our fees are similar to other medical offices; however, we offer discounts based on yearly income and family size. Total cost for services depends on the testing and treatment needed after adjusting for any qualifying discounts. Payment is expected on the day of your appointment. When you schedule your appointment, be sure to ask PrairieStar staff what your expected payment will be.
PrairieStar receives some of our funding from grants. In order to keep this grant money, we request your proof of income each year. You will not receive a discount unless you provide us with proof of income. Acceptable proof of income includes the following:
- Current Federal Tax returns
- Current Paycheck Stubs
- Current Social Security Statements
- Current W2's
If you do not have any of these forms, please inquire at the front desk or with patient accounts to see what other proof of income is acceptable.
Insurance Billing: Medicaid, Medicare and most commercial insurance policies are accepted and billed directly by PrairieStar. We accept most forms of health and dental insurance and may be able to bill your insurance company directly. Please bring your insurance card with you to each visit to assist us with this process.
Statements: PrairieStar sends monthly statements to patients when payments made at the time of the visit do not cover all the charges incurred during the visit. This might happen when additional services are received, insurance does not pay the entire amount we expected, or for various other reasons. Payment is due when statements are received. Monthly payments toward outstanding charges are expected.
